Nurse Practitioner – Residential Aged Care | Newcastle, NSW Join a well-established organisation providing quality primary care services to residents in Aged Care Facilities across the Newcastle region. This is a permanent, full-time opportunity (5 days per week) for an experienced and autonomous Nurse Practitioner looking to make a real impact in aged care. About the Role: You’ll work closely with the organisation’s Senior Medical Officer and General Practitioners to deliver high-quality, patient-centred care and reduce unnecessary hospital admissions. You’ll manage your own caseload, visiting assigned RACFs weekly and coordinating care between residents, families, GPs, and facility staff. Key Responsibilities: Conduct clinical assessments, care plans, and health reviews for aged care residents Undertake examinations, order investigations, and implement management plans Provide preventive and primary health care Coordinate with GPs and aged care staff to ensure continuity of care Contribute to medication reviews and annual health assessments Maintain documentation and collaborate closely with the care team About You: Current AHPRA registration and Nurse Practitioner endorsement Eligible for a Medicare Provider Number Strong clinical judgement and ability to work autonomously Excellent communication and collaborative skills Professional indemnity insurance and unrestricted Australian work rights What’s on Offer: Full-time, ongoing role (5 days per week) Collaborative, team-based model with GP and SMO support Rewarding aged care environment with continuity of patient care Opportunities for remote work once relationships are established Sound like you?
